Bust. After 15 miles or so behaving it jumped out again while I was pottering along at 50mph. I've covered everything with the linkage, it would be doh! moment to fit a different box and find it did the same due to a £5 piece of plastic or similar. Yeah that would be a nightmare, have you spoken to bears or cog box to see if they have any ideas on possible causes? No but Rob knows a lot about them too. There are a few possible causes, one is easy fix even I could do... But the only way to find out is engine and box out so to cover the eventuality that it can't be easily fixed there and then I need a replacement ready. Eventually I'll have two working boxes and pass one on. That way I won't be off the road or much out of pocket, hopefully.

Good plan What I thought had possibilities of extreme money wasting annoyance would be buy a surcharged one from bears, then having to send them my otherwise nice gearbox just needing a new sub £50 nosecone.

When I suggested that option I was under the impression your gearbox was definitely broken! Oh it is, but what exactly is broken is the question. There is a common problem that can be quickly sorted with a coke can but gearbox rebuilders ( who do this and take the money) wouldn't want that to be common knowledge.

Once the shift coupler started pushing it out of gear by hitting the beam it started damaging the gearbox every time it popped out.
Now its probably got a permanent slope on the teeth or rounded off corners. I dont know if the ratios use a prime number of teeth on one side of the meshed 4th gear pair.. but if not there may be a few fixed patterns of meshing of teeth. If thats the case you may have like a Russian Roulette of 4th gear meshing : some sets of teeth mesh well others are knackered. Some days you never hot the damage others do.

Indeed, but of course I was in the dark until it jumped out. It's only done it about 20 times total. Anyway, my van now has an extra box loose in the back.
